Sleep Laboratory Studies

After your sleep study is scheduled, our billing office will contact your insurance company to ensure coverage and determine any out-of-pocket costs for which you are responsible. We will make every attempt to verify your benefits and work with your insurance company to obtain prior approval, but we can not guarantee coverage in advance, as insurance companies reserve the right to deny coverage. Your copayment is due at the time of your study, and any unmet deductibles will be billed to you following insurance payment. We accept cash, personal check, Visa, and Mastercard. If this is not a covered service under your insurance plan, we can work out a payment plan for you if payment in full at the time of your visit is not possible. Please work this out with our billing specialist prior to your visit.

Referrals

Please remember to bring your referral to your appointment if your insurance company requires one. If you do not have it with you at the time of service, you will be asked to sign a Referral Waiver form and will be charged in full for the services provided. A refund of the fee minus any copayment/deductible will be given if an appropriate referral is received within 3 business days.

Late Cancellation Fees

Please provide us with advanced notice if you are unable to keep an appointment. This allows us to offer appointments to other patients. Late cancellation fees will be charged if advanced notice is not given, and they are YOUR responsibility. They can not be billed to insurance.

• There is a $100 late cancellation for sleep studies cancelled with less than 72 hours notice.

• There is a $50 late cancellation fee for office visits cancelled with less than 48 hours notice.
